Under Data Protection legislation the church Charity Trustees of Eastleigh Baptist Church (the Church) are the Data Controller and the Church Secretary (Tony Harrison) acts as our Data Protection Trustee. Our Data Protection Leads are our Church Operations Manager (Emma Leggett) and one of the Charity Trustees (Paul Wilton).
We are collecting this information to enable the church to run our Holiday Club safely and to ensure we can contact you (or other nominated adult) in case of an emergency. If you are unable to supply the information requested then we will be unable to accept your child at our Holiday Club.
Data Protection legislation allows us to process this information as we regard it as being in the Church’s legitimate interest. The Church collects this information through a third party cloud based system (Jotform Inc). The Church has a legal agreement in place with Jotform Inc to ensure your data is processed under the same Data Protection legislation.
The Church will ask for your consent to take photographs of your child during Holiday Club. Your child’s name will not be recorded against the photographs. The Church has a photograph policy which you can view here. You have the right to withdraw consent at any time at which point any photographs of your child will be deleted. To withdraw your consent please use the contact email or telephone number below.
The information you supply on the form will be held both in paper form and electronically. In paper format the information will be kept in a securely locked cupboard in the Church office. In data format the information is held securely on the Church’s password protected computer system and securely on a password protected, third party cloud based system (Jotform Inc). Only the Church’s Administration Team, the Holiday Club Leaders and Jotform will have access to this information. The forms and electronic records will be kept in line with our retention policy which can be viewed here.
